Michael,
I've overseen the development of online math courses at two schools now! Fun. One thing that works well is what they do at LeTourneau. Blackboard with textbook (and publisher's website) and MyMathLab. Seems to work well.
Our Math for Liberal Arts teacher felt the textbooks used over the years were deficient in what needed to be taught so she wrote her own textbook! It is not yet published (though she is contemplating it), but a PDF is provided in the course with a course fee charged at registration that in turn gets sent to the teacher as a royalty. Works quite well.
If you have someone who knows how to write the formulae, Blackboard has formula writing capabilities embedded in it.

additional supplement. Students do a number of assignments in My Math
Lab: http://www.mymathlab.com/
Discussions take place in Blackboard, but the assignments with heavy
equations and math work happen in My Math Lab.

Michael,
At Toccoa Falls College, we use Moodle and our assignments and exams are faxed to the Online office currently since our instructor is oncampus. We use Video and PowerPoint with audio for group teaching, but we employ DIMDIM live chatroom with a Bamboo live tablet for individualized help in realtime. All other course expectations are met with Moodle's tools. Our math course also uses a textbook.
